Case Manager RN - PRN
Position Summary: This position supports the mission of The Medical Center of Aurora and Centennial Medical Plaza optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The RN Case Manager is responsible and accountable for coordination of services through an interdisciplinary process that provides a clinical, financial, and psychosocial approach through the continuum of care. Balances individually identified patient and family needs with the efficacious and cost effective utilization of resources. Through concurrent clinical case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, clinical resource consumption, progressive care management, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Quality and Risk Management issues and education needs will also be monitored for and reported to the QRM DEPARTMENT as a part of the CQI process. When need arises, will perform other duties as assigned by supervisor.

A. Licensure/Certification/Registration: Current licensure in the State of Colorado as a Registered Nurse; BLS – Healthcare provider. Special certification in case management preferred.
B. Education: Graduate of an accredited school of nursing. BSN Preferred.
C. Experience: 5 or more years clinical experience required. 1 to 3 years Case Management experience preferred
D. Special Qualifications: Ability to assess information, apply critical thinking processes to determine course of action, define expected outcomes and evaluate effectiveness of plan through continuous assessment of the patients progress. Ability to compile, evaluate and report statistics to members of the team and utilize the information to facilitate process improvement activities. Strong oral/written communication skills are necessary for meeting facilitation, educational presentations, team collaboration and patient interviews. Requires excellent human relations skills and strong analytical/organizational skills. Must have ability to perform multiple tasks simultaneously and work in high stress environment. Position requires flexibility and cross coverage.
Degree of supervision required: Involves general guidance and direction by the Department Director. Employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures.
Populations and Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards. (The skills and knowledge needed to provide such care may be gained through education, training or experience.)
